About the role

The Director of Care Coordination manages day-to-day operations of the Care Coordination department at Peterson Health. This includes overseeing patient care activities, ensuring smooth transitions of care for patients within the hospital, providing leadership to all staff, ensuring quality care, and managing change.

Responsibilities

• Oversee patient care activities of the department, ensuring smooth transition of care for patient within Peterson Health.
• Provide leadership to all staff, ensuring quality care and managing change.
• Carry out all duties while maintaining compliance, confidentiality, and promoting the mission and philosophy of the organization.

Requirements

• Required Education: Bachelor of Science in Nursing
• Preferred Education: Master of Science in Nursing or Master of Business Administration
• Required Skills: Leadership experience, basic computer and keyboarding, Registered Nurse, Licensed in the state of Texas or Compact State, Certification in Case Management
• Required Experience: Three years acute care nursing experience, Leadership experience, Case Management experience

Qualifications

• Compliance with all regulatory requirements and organizational policies.
• Ability to work independently and as part of a team.
• Strong communication and interpersonal skills.
