Job Summary and Qualifications

The Facility Case Management Director at HCA Florida West Hospital oversees and evaluates departmental operations, allocates resources, and ensures compliance with policies and SOPs. This role requires a bachelor's degree in nursing, health administration, or business administration, or a master's degree in a related field, and at least three years of experience in acute care hospital case management, with two years of experience in case management leadership.

Responsibilities

  • Directs and evaluates departmental operations, including the case management model, staffing, use of information technologies, onboarding, and staff competencies.
  • Allocates resources to effectively staff the department and meet productivity and quality goals.
  • Oversees the adherence to care coordination and discharge planning processes, ensuring timeliness, quality, and proper documentation.
  • Works with the Facility CFO and Division CM Leader to achieve established goals and expectations.
  • Affirms the department’s performance by evaluating operational processes, monitoring performance through analyzing data, and implementing sustainable performance improvement activities.
  • Ensures compliance with policies and SOPs.
  • Establishes working relationships with key stakeholders, including the CMO, CNO, ancillary service leaders, Ethics & Compliance, and Legal.
  • Performs other duties as assigned.

Qualifications

  • Bachelor's Degree in Nursing, Health Administration, or Business Administration, preferred.
  • Registered Nurse (RN) or LCSW (Clinical Social Worker).
  • No travel required.
  • Three years of experience in overall acute care hospital case management, preferred.
  • Two years of experience in case management leadership, preferred.

The ideal candidate must be a self-starter, capable of managing multiple priorities, working with minimal supervision on projects and activities, demonstrating tact and diplomacy in situations of conflict and controversy, and adapting quickly to change while coordinating efforts across multiple stakeholders.
