About the role
Responsible for providing direct support services to individuals with disabilities in a community setting.
Responsibilities
- Provide daily personal care and hygiene assistance
- Assist with medication management and health monitoring
- Facilitate social activities and community integration
- Maintain safety and security of individuals served
- Document progress and communicate with families and professionals
Requirements
- High school diploma or equivalent
- Valid driver’s license and reliable transportation
- Passing score on a background check
- Experience working with individuals with disabilities preferred
Qualifications

- Ability to work independently and as part of a team
- Good physical stamina and flexibility
