Direct Support Professional - Community Participation Supports
About the role
As a Direct Support Professional focused on Community Participation Support, you will give direct client care and programming to individuals in the HCBS program. You will participate in the development and implementation of the Individual Service Plans, requiring work activities in the facility or in the community with single or multiple participants who are ambulatory and non-ambulatory. You will also ensure each individual participates in social/leisure/recreational activities based on individualized plans and provide social, emotional, and environmental supports to ensure the happiness, health, and safety of the participants.
Responsibilities
- Develop and implement Individual Service Plans
- Provide direct client care and programming
- Work with single or multiple participants in the facility or community

Requirements
- At least 18 years of age
- A High School Diploma or equivalent
- A valid PA driver’s license and a good driving record
