About the role
As the Lead Direct Care Professional, you will serve as both a caregiver and a leader. You will mentor and support Direct Care Professionals while helping ensure every individual receives compassionate, person-centered care that reflects their unique needs, preferences, and aspirations. Every day brings opportunities to make a difference by helping individuals navigate daily life while building relationships based on trust, respect, and understanding.
Responsibilities
- Lead, mentor, and support Direct Care Professionals to ensure consistent, high-quality care.
- Provide direct support with personal care, medication administration, daily living activities, and community engagement.
- Coordinate appointments, transportation, and activities that encourage participation in the community.
- Help maintain accurate documentation and ensure compliance with care plans, medical orders, and regulatory requirements.
- Support meal planning and preparation while ensuring individual dietary preferences and needs are respected.
- Participate in the development of Individual Plans of Service (IPOS) and advocate for the needs and goals of residents.
- Maintain a safe, clean, and welcoming home environment.
- Assist with quality assurance activities, emergency preparedness, medication audits, inventories, and other operational responsibilities that help the home run smoothly.
Requirements
- Associate degree in a related field preferred, or a professional license such as CNA—not required.
- Previous experience in direct care, healthcare, or a related support environment preferred.
- Leadership, mentoring, or supervisory experience preferred.
- Strong communication, problem-solving, and organizational skills.
- A commitment to person-centered care and maintaining the highest standards of quality and compassion.

Pay
Hourly rate of $20.50.
Schedule
- Full-time, Afternoon Shift (3:00pm - 11:00pm), Monday - Friday.
- On-call rotation approximately 1 or 2 weekends per month.
- Overtime may be necessary based on operational and resident needs.
