Essential Duties and Responsibilities

  • Aid in improving patient health through clinical and operational processes.
  • Coordinate temporary housing arrangements for patients.
  • Effectively utilize medical supplies during patient care.
  • Perform assigned duties to meet patient care or operational needs.

Outcomes

  • Identify and address psychosocial concerns of patients and families.
  • Provide patient and family education sessions.
  • Contribute to achieving program targets for patient outcomes.

Operational Readiness

  • Comply with federal, state, and local laws and regulations.
  • Follow company policies and procedures, as well as applicable laws and regulations.
  • Act as a patient advocate with relevant local, state, and federal agencies.
  • Complete initial assessments and KDQOL evaluations according to company and regulatory guidelines.
  • Participate in interdisciplinary team meetings for patient assessments and care planning.
  • Identify and connect patients with social services and funding sources.
  • Stay informed about professional organizations related to kidney disease.
  • Use personal protective equipment as necessary.
  • Be familiar with emergency procedures.

Partnerships

  • Offer information and referrals to patients as needed.
  • Attend and contribute to QAPI activities.
  • Organize and maintain patient/family support groups.
  • Maintain collaborative relationships with the Medical Director and physicians.
  • Build positive relationships with area hospitals, agencies, vendors, and the community.
  • Promote guest customer service standards and foster a team-oriented environment.

Staff Development/Retention

  • Participate in staff meetings as required.
  • Attend in-service and continuing education programs.
  • Train staff on patient psychosocial care.
  • Lead teams and promote teamwork.

Qualifications/Requirements

  • Master's degree in Social Work from an accredited institution, with LCSW preferred.
  • Current licensure in the applicable state, with exceptions for Arizona, Pennsylvania, and Guam.
  • Maintain a current and active license in good standing.
  • Previous experience in providing social services to dialysis patients is preferred.
  • Demonstrated proficiency in the English language, both verbal and written.
  • Basic computer skills, including proficiency in Microsoft Office and USRC clinical applications.
  • Meet any additional practice requirements for the applicable state.
