Essential Duties and Responsibilities
- Aid in improving patient health and reducing hospitalizations and missed treatments through clinical and operational processes.
- Coordinate temporary living arrangements for patients.
- Effectively manage and utilize medical supplies.
- Perform assigned duties to meet patient care or operational needs.
Outcomes
- Identify and address psychosocial issues affecting patients and their families.
- Provide patient and family education sessions.
- Contribute to achieving program targets for patient outcomes aligned with quality care goals.
Operational Readiness
- Comply with federal, state, and local laws and regulations, including those related to healthcare practice acts.
- Follow company policies and procedures, as well as applicable state and federal laws and regulations.
- Act as a patient advocate by communicating with local, state, and federal agencies as necessary.
- Complete initial assessments of new patients' psychosocial needs in accordance with state and CMS regulations and company policy.
- Accomplish KDQOL assessments in line with company policy.
- Participate in interdisciplinary team meetings for patient assessments and care planning.
- Identify relevant social services and resources for patients, including financial assistance.
- Stay updated on professional organizations pertinent to kidney disease.
- Use personal protective equipment as needed during patient interactions.
- Be familiar with emergency protocols and procedures.
Partnerships
- Offer information and referrals to patients as needed.
- Attend and contribute to monthly QAPI activities.
- Coordinate the creation and maintenance of patient/family support groups.
- Maintain cooperative relationships with the Medical Director and physicians.
- Build and sustain positive relationships with area hospitals, agencies, vendors, and the broader community.
- Promote high-quality customer service standards across the organization.
- Collaborate with staff to enhance teamwork and foster a supportive work environment.
Qualifications/Requirements
- Master's Degree in Social Work from an accredited institution (CSWE) and LCSW certification preferred.
- Current licensure in the applicable state is required, except for Arizona, Pennsylvania, or Guam.
- Licenses must remain valid and in good standing.
- Previous experience in social services for dialysis patients is beneficial.
- Strong communication skills, both verbal and written, are essential.
- Basic computer skills, including proficiency in Microsoft Office (Word, Excel, Outlook), are necessary.
- Must meet any additional practice requirements for the applicable state.
