Dialysis Clinical Social Worker

About the role

This position will cover care of patients at three clinics to include: Dover Dialysis, Millersburg Dialysis, and Wooster Dialysis.

Responsibilities

• Support the psychosocial health and wellbeing of dialysis patients.
• Provide interventions as needed and complete mandatory assessments, screenings, and education.
• Build meaningful and long-term relationships with patients and their families in an outpatient setting.
• Work independently while benefiting from a supportive team environment.
• Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
• Help patients understand their rights and responsibilities.

Requirements

• Masters degree in Social Work (MSW) with a specialization in clinical practice.
• Licensed Social Worker (LSW) license in Ohio.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
• Functional proficiency with DaVita specific applications within 60 days.

Pay

Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies, and experience.

Schedule

The schedule will be 40 hours/week within - Monday - Friday 8:00am - 4:30pm (some flexibility/variances depending on patient needs).
