Essential Duties and Responsibilities

  • Aid in improving patient health and reducing hospitalizations and missed treatments through clinical and operational processes.
  • Coordinate temporary housing arrangements for patients.
  • Effectively manage and utilize medical supplies.
  • Perform assigned duties to meet patient care or operational needs.

Outcomes

  • Identify and address psychosocial concerns of patients and their families.
  • Provide patient and family education.
  • Achieve program targets for patient outcomes aligned with quality care goals.

Operational Readiness

  • Comply with federal, state, and local laws and regulations, including health care professionals practice act requirements.
  • Follow company policies and procedures, as well as applicable state and federal laws and regulations.
  • Act as a patient advocate with local, state, and federal agencies.
  • Conduct initial assessments of new patients' psychosocial needs according to state/CMS regulations and company policy.
  • Complete the KDQOL assessment in accordance with company policy.
  • Participate in interdisciplinary team activities for patient assessment and care planning.
  • Identify relevant social services and resources for patients, including funding sources.
  • Stay informed about professional organizations related to kidney disease.
  • Use personal protective equipment as necessary.
  • Be familiar with emergency procedures.

Partnerships

  • Offer information and referrals as needed.
  • Attend and contribute to monthly QAPI activities.
  • Develop and maintain patient/family support groups.
  • Maintain collaborative relationships with the Medical Director and physicians.
  • Build positive relationships with area hospitals, agencies, vendors, and the community.
  • Promote guest customer service standards and foster effective relationships throughout the organization.

Staff Development/Retention

  • Participate in staff meetings as required.
  • Attend in-service and continuing education programs in compliance with company policy and procedure.
  • Educate staff on patient psychosocial care.
  • Lead teams and promote teamwork among staff members.

Qualifications/Requirements

  • Master's Degree in Social Work from an accredited institution (CSWE) and LCSW preferred.
  • Current licensure in the applicable state is required unless employed in AZ, PA, or Guam.
  • Licenses must be maintained as current and in good standing (see addendum for specific licensing requirements).
  • Previous experience in providing social services to dialysis patients is preferred.
  • Demonstrated proficiency in the English language and ability to communicate verbally and in writing.
  • Basic computer skills, including Microsoft Office (Word, Excel, Outlook), are required within 90 days of hire.
  • Must meet any practice requirements for the applicable state.
