About the role
As a Dietitian, you will be a vital member of an interdisciplinary team, delivering high-quality nutritional care to patients with kidney disease. You will collaborate with healthcare providers, review lab results, and recommend therapeutic diets tailored to individual needs, preferences, and treatment changes.
Responsibilities
- Develop and adjust patient nutrition plans based on current, science-based practices in consultation with physicians.
- Educate patients on nutrition topics such as fluid, sodium, potassium, and CKD-Mineral and Bone Disorder management.
- Conduct patient assessments, care planning, and participate in interdisciplinary care conferences.
- Collaborate with medical staff, facility teams, and community partners to ensure coordinated care.
- Support clinical and operational processes aimed at improving patient outcomes and maintaining quality assurance programs, including QAPI activities.
Qualifications & Skills
- Current Registered Dietitian (RD/RDN) certification with proof of registration.
- Minimum of one year of clinical nutrition experience.
- Valid dietitian license in the applicable state or equivalent certification as required.
- Excellent verbal and written communication skills in English.
- Basic computer proficiency, including Microsoft Office; all clinical applications proficiency required within 90 days.
