Role Overview The Diabetes Educator/Dietitian coordinates patient diabetes self-management and nutritional care through assessment, education, and documentation. They provide age-specific education to patients, families, and healthcare teams, fostering continuity of care across all phases. This role involves developing personalized nutrition care plans, implementing diabetes self-management education, and evaluating patient outcomes to ensure quality service delivery. The position requires collaboration with multidisciplinary teams, participation in quality improvement initiatives, and community engagement. The educator also serves as a resource on nutrition and diabetes management, delivering presentations and supporting professional development. Key Responsibilities Assess patient needs and develop tailored education and nutrition care plans.Provide individualized and group education on diabetic diets, eating behaviors, and community resources.Document patient encounters and monitor outcomes using evidence-based practices.Participate in quality improvement, policy development, and clinical research.Collaborate with healthcare teams and contribute to staff education and mentoring.Engage in ongoing professional development, maintain competencies, and support community events. Qualifications & Skills Knowledge of diabetes management and nutrition care standards, including ADA and Joint Commission guidelines.Strong assessment, educational, and communication skills, with empathy and cultural sensitivity.Ability to work independently and as part of a multidisciplinary team.Commitment to professional growth, evidence-based practice, and continuous learning.Experience in developing and implementing care plans, documenting care, and participating in quality initiatives.
