Diabetes Educator RD

Role Overview: The Diabetes Educator/Dietitian coordinates patient diabetes self-management and nutritional care through assessment, education, and documentation across all care phases. Key Responsibilities Develops and implements individualized nutrition care plans aligned with evidence-based standards.Provides patient education on diabetic diets, food choices, eating behaviors, and community resources, tailored to age-specific needs.Collaborates with patients, families, and healthcare teams to assess DSMES (Diabetes Self-Management Education and Support) needs and set goals.Performs medical nutrition therapy, including screening, assessment, diagnosis, intervention, and monitoring, utilizing the Nutrition Care Process.Documents patient encounters, evaluates outcomes, and participates in quality improvement initiatives.Ensures compliance with regulatory standards, ethical practices, and organizational policies.Shares expertise through presentations, community events, and mentorship of interns and staff.Supports professional development through ongoing education, self-assessment, and participation in research and team activities. Qualifications & Skills Ability to independently perform clinical functions and adapt education to diverse patient needs.Strong knowledge of nutrition, diabetes management, and evidence-based practices.Excellent communication, teamwork, and documentation skills.Commitment to continuous learning and professional growth.
