Developmental Disabilities Service Coordinator - Fremont
About the role
The Department of Health and Human Services (DHHS) is seeking a Service Coordinator within the Division of Developmental Disabilities (DDD) to provide funding and oversight of services to individuals with disabilities, including intellectual or developmental disabilities, children with special health care needs, or the aging.
Responsibilities
- Manage diverse relationships and be action-oriented
- Communicate effectively and stay organized
- Serve as a case manager for people eligible for a Waiver
- Coordinate and oversee the delivery of services
- Conduct assessments and eligibility determinations
- Develop plans of services and supports
- Provide appropriate referrals
- Maintain monitoring of service provision
- Ensure the health, welfare, and safety of those we support
Requirements / Qualifications
- Minimum Qualifications: Bachelor’s degree and professional experience in education, psychology, social work, sociology, human services, or a related field
- Experience in services or programs for people with intellectual or other developmental disabilities
- Preferred Qualifications: Experience with developing person-centered plans, completing assessments, habilitation, program implementation, team and meeting facilitation, experience in Word, Excel, Outlook, Nfocus and Therap, and experience in services or programs for people with special healthcare needs
- Other: Valid driver's license and immediate access to a reliable vehicle
