Developmental Disabilities Service Coordinator - Lincoln
Job Description
The Department of Health and Human Services (DHHS) is seeking a Service Coordinator within the Division of Developmental Disabilities (DDD) to provide funding and oversight of services to people served within the Home and Community-Based Services (HCBS) waiver program. This includes individuals with disabilities, intellectual or developmental disabilities, children with special health care needs, or the aging.
Requirements / Qualifications
- Minimum Qualifications: Bachelor’s degree and professional experience in education, psychology, social work, sociology, human services, or a related field, with experience in services or programs for people with intellectual or other developmental disabilities.
- Preferred Qualifications: Experience with developing person-centered plans, completing assessments, habilitation, program implementation, team and meeting facilitation, experience in Word, Excel, Outlook, Nfocus and Therap, and experience in services or programs for people with special healthcare needs.
- Other: Possess a valid driver's license and have immediate access to a reliable vehicle. A state vehicle may be provided dependent upon office location.
