Delta: Care Coordinator

About the role

PeopleCare Health Services is hiring a Care Coordinator to manage client caseloads and coordinate quality home care services across various counties in Western Colorado.

Responsibilities

• Manage an assigned caseload of clients
• Coordinate care with clients, families, caregivers, and community partners
• Complete intake assessments and required client visits
• Support caregiver scheduling and coverage needs
• Help address client or staff concerns promptly and professionally

Qualifications

Healthcare, case management, or caregiving experience preferred

Strong communication, organization, and problem-solving skills

Comfortable working independently, in a fast-paced environment, and as part of a team

Work Setting

This role includes working from home, attending occasional in-office meetings, visiting client homes, and traveling throughout the field. Travel is required up to 50% of the time.

Pay

Pay Range: $20-$25 an hour
