DDS Uxbridge House - Licensed Practical Nurse (20 Hours)

Responsibilities

• Oversee medication ordering, counts, and refills following physicians' written orders.
• Maintain and monitor individual's medical status and transport, or delegate transportation to and from medical appointments.
• Advocate for individuals with physicians and other service providers.
• Collaborate with the staff team to implement health and wellness standards, including the development of nutritious meals.
• Attend and participate in Individualized Support Plan (ISP) meetings and complete necessary documentation for the ISP's.

Requirements

• A High School Diploma, GED, or equivalent, is required.
• A Certified Licensed Practical Nurse (LPN) license in Massachusetts is required.
• Excellent assessment, documentation, and verbal communication skills are essential.
• A valid driver's license and acceptable driving history are required.
• Experience working with adults with disabilities is preferred.
