Description and Responsibilities

The Licensed Practical Nurse works in close collaboration with the Registered Nurse to ensure the medical needs of people supported are met. This position involves acting as a liaison between the program and other stakeholders, including family members and other medical providers.

  • Oversee medication ordering, counts, and refills following physicians' written orders.
  • Monitor individual's medical status and transport, or delegate transportation to and from medical appointments.
  • Advocate for individuals with physicians and other service providers.
  • In collaboration with the staff team, ensure health and wellness standards are being implemented including development of nutritious meals.
  • Attend all Individualized Support Plan (ISP) meetings and complete necessary documentation for the ISP’s.

Qualifications

  • A High School Diploma, GED, or equivalent, required.
  • A Certified Licensed Practical Nurse in Massachusetts, required.
  • Excellent assessment, documentation, and verbal communication skills, required.
  • A valid driver’s license and acceptable driving history, required.
  • Experience with adults with disabilities, preferred.
