About the Role

This position involves conducting comprehensive assessments, developing service plans, and coordinating care for clients. The role requires advocacy, crisis intervention, and ongoing monitoring to ensure clients receive necessary services and support.

Responsibilities

• Conducts and documents comprehensive assessments of client needs.
• Participates in the development of client service plans based on assessments.
• Arranges for delivery of services identified in the assessment.
• Assists clients in accessing services through referrals or linkages.
• Monitors client progress by tracking appointments, following up on services, and performing periodic reassessments.
• Performs advocacy activities on behalf of clients (e.g., expediting entitlement payments, encouraging self-advocacy).
• Prepares and maintains case records in accordance with organizational and regulatory standards.
• Provides case consultation with service providers to determine client status and progress.
• Communicates with supervisors about case management, resource collaboration, and development of new resources.
• Provides psychosocial education to clients and family members to enhance understanding of illnesses or conditions.
• Performs crisis intervention, including emergency referrals and coordination of services.
• May serve as Primary Case Coordinator (PCC), with responsibilities varying based on skill and autonomy.

Requirements

• Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or a related field approved by Medicaid or applicable funding source.
• Minimum of zero years of experience in case management or service coordination with a Bachelor’s degree.
• Working knowledge of mental and physical characteristics of assigned population groups, human behavior, treatment options, and related issues.
• Familiarity with SCS programs, services, policies, and procedures.
• Understanding of applicable regulations and licensure standards.
• Strong interpersonal, written, and oral communication skills.

Physical Demands

• Lifting of up to 20 pounds maximum, with frequent lifting or carrying of items up to 10 pounds.
• Significant periods of walking or standing.
• Potential exposure to blood, bodily fluids, or tissue.
• May require 24-hour on-call responsibilities.
• Reliable transportation for frequent travel.
