Day Support Professional, Community Services

About the role

The Day Support Professional, Community Services provides high-quality habilitation services to individuals in their residences or within community settings. This role supports individuals in achieving their personal goals, fostering independence, and enhancing their community integration through person-centered services.

Primary Responsibilities And Essential Functions Of Position

• Advocate for, encourage, guide, and teach individuals in expressing personal choice
• Ensure community integration and assist with personal hygiene care, toileting, lifts and transfers, dining, dressing, and meal preparation
• Coach and encourage individuals to develop daily living skills
• Provide a clean, safe, and comfortable environment
• Collaborate with other staff to implement and record care plans
• Administer medication as instructed
• Participate in staff meetings, in-service training, and on-site workshops
• Attend external training and workshops as required
• Be prepared to act in emergency situations to ensure the health and safety of individuals

Qualifications

• High school diploma/GED required
• CNA certification or DSP certificate from an accredited organization preferred
• One year or more work experience with individuals with developmental disabilities preferred
• Valid NYS driver license preferred but not required
