Career Summary

We have an opening for a Day Habilitation Lead. This team member will contribute to a great life for individuals by supporting their health and skill development and retention. In addition, the role will provide or assist with care, delivery of services, and community integration of Day Hab participants.

About the Role

This position is responsible for leading group and individual activities to build community integration, independence, and social skills; implementing Individual Care Plans; administering medications and treatments per policy; ensuring dignity, privacy, and positive behavior support; planning and adapting activities/field trips for accessibility; coordinating transportation and community resources; maintaining a safe, therapeutic, and clean environment following infection control and safety standards; mentoring and supporting staff, ensuring continuity of care, attending meetings, and fostering a positive team culture; communicating professionally with families, supervisors, and staff regarding updates and changes; maintaining accurate, timely records for care, medications, observations, and adverse events; reviewing staff documentation; completing required forms and ensuring compliance with organizational and regulatory standards; identifying training needs, participating in continuing education, mentoring others; following ChildServe policies, CARF standards, confidentiality, and safety procedures; performing additional duties as assigned.

Responsibilities

  • Care Delivery: Leads group and individual activities to build community integration, independence, and social skills; implements Individual Care Plans; administers medications and treatments per policy; ensures dignity, privacy, and positive behavior support.
  • Activity & Environment Management: Plans and adapts activities/field trips for accessibility; coordinates transportation and community resources; maintains a safe, therapeutic, and clean environment following infection control and safety standards.
  • Team & Communication: Mentors and supports staff, ensures continuity of care, attends meetings, and fosters a positive team culture; communicates professionally with families, supervisors, and staff regarding updates and changes.
  • Documentation & Compliance: Maintains accurate, timely records for care, medications, observations, and adverse events; reviews staff documentation; completes required forms and ensures compliance with organizational and regulatory standards.
  • Professional Development & Policy Adherence: Identifies training needs, participates in continuing education, mentors others; follows ChildServe policies, CARF standards, confidentiality, and safety procedures; performs additional duties as assigned.

Requirements

  • Education & Experience: Four-year degree preferred; minimum one year working with individuals with disabilities.
  • Technical & Physical Skills: Proficient in medication administration, behavior support, g-tube feedings, seizure management, and lifting/transferring; able to lift up to 35 lbs and perform frequent physical tasks.
  • Behavior & Communication: Maintain calm under changing conditions and elevated behaviors; communicate effectively in English, both verbally and in writing.
  • Compliance & Driving: Complete background checks; meet ChildServe driver criteria, obtain Chauffeur license if needed, and provide a well-maintained personal vehicle for transportation.
  • Leadership & Teamwork: Ability to lead teams, promote collaboration, hold staff accountable, and support skill development.

Schedule/Where You’ll Be

$19/hr. M-F 8:30am – 3:30pm Johnston, IA Campus

What You’ll Need

Education & Experience: Four-year degree preferred; minimum one year working with individuals with disabilities.

Technical & Physical Skills: Proficient in medication administration, behavior support, g-tube feedings, seizure management, and lifting/transferring; able to lift up to 35 lbs and perform frequent physical tasks.
