About the role
Waystone is currently seeking a dedicated Case Manager for its Day Habilitation Program located in Haverhill, MA. The Case Manager oversees and coordinates all support services for individuals served by Waystone.
Responsibilities
- Develop service plans, implement goals, conduct assessments, maintain accurate records, ensure documentation compliance, track attendance, and complete required submissions through Electronic Health Records and related systems.
- Train and guide staff to ensure support plans are carried out effectively and high-quality services are consistently delivered.
- Coordinate services aligned with DHSP/ISP and Behavior Support Plans to promote choice, inclusion, dignity, independence, and skill development.
- Document meetings for case conferences, support plan reviews, and trainings.
- Identify concerns proactively and collaborate with Supervisor/Director for person-centered solutions.
- Provide IDT updates with data-informed feedback on medical status, goal progress, and safety concerns.
- Implement plans including crisis plans, behavior treatment plans, and medical protocols.
- Provide personal care aligned with medical, physical, and psychological needs.
- Understand individual profiles to support positive learning environments.
- Communicate issues affecting health, safety, or well-being.
- Collaborate with teams including staff, families, departments, and funding sources.
- Develop curriculum aligned with day habilitation need areas.
- Advocate for community engagement in recreational, vocational, and therapeutic activities.
- Incorporate feedback to support decision-making and skill development.
- Maintain documentation including attendance, incidents, accidents, restraints, case notes, and health records.
- Auxiliary with audits including binder audits and training compliance.
- Promote teamwork and provide person-specific training/support.
- Respect rights in all interactions.
- Maintain confidentiality for all individual information.
- Complete mandatory training including CPR, First Aid, and Waystone-required courses.
- Report injuries immediately and complete follow-up documentation; ensure MassHealth compliance.
- Participate in QA inspections, audits, and reviews.
- Contribute to newsletter with team collaboration.
- Provide transportation as requested.
- Maintain a safe environment by reporting issues promptly.
Qualifications
- Bachelor’s degree in Human Services or related field with at least three (3) years of experience within the field of ID/DD or High School Diploma or GED with at least five (5) years of experience within the field of ID/DD.
- Bilingual proficiency in English and Spanish preferred.
- Minimum of two (2) years with case management, case reviews, development and implementation of goals and group activities, preferably in a related setting.
- Experience working with adults with developmental disabilities for a minimum of 1 year required.
- Familiarity with the Positive Behavior Supports model by DDS is a plus.
- Strong communication, organizational, and interpersonal skills with the ability to manage multiple priorities in a fast-paced environment.
- Proficient with Microsoft Office and general office technology; able to handle confidential information with professionalism and sound judgment.
- Valid Driver’s license with a minimum of 1 year’s driving experience required.
- Regular and predicable attendance.
