Under the direction of the Day Habilitation Assistant Director, the Day Hab Support Coordinator supervises assigned Day Habilitation Disabilities Specialists, including performance evaluations, and ensures all staff action plans, goals, and training are implemented with individuals.
Responsibilities
- Ensure staff action plans, goals, and all other plans are implemented with individuals and that all staff are trained in these plans.
- Monitor Precision Care to ensure all required meetings have been held, timelines met, and documents uploaded.
- Ensure compliance with OPWDD/Medicaid requirements and standards for meetings and billing.
- Participate in quarterly medical, clinical, Day Program, and Life Plan meetings; ensure recommendations are implemented as prescribed.
- Oversee coordination and follow-up of program participants’ needs and goals through ongoing observation of activities at the program and in the community.
- Maintain direct contact with assigned program participants to understand their behavioral, social, and functional needs.
- Create and maintain a projected schedule for semi-annual and annual Life Planning Team meetings with care managers, day program team, employment representatives, respite, residence, advocates, and family members.
- Ensure Day Habilitation Support Professional Specialists complete written minutes for Staff Action Plans, reflecting progress toward goals and supports.
- Train and ensure implementation of goal-related and valued outcome-related activities at the program center and in the community.
- Provide support with appropriate supervision, considering each individual’s needs and routine.
- Review, correct, and approve staff action plans, including goals, valued outcomes, and community inclusion activities.
- Certify Staff Action Plans semi-annually, annually, or as necessary.
- Ensure Staff Action Plans align with at least two CQL Valued Outcomes as identified in the individual’s Life Plan.
- Maintain current and accurate Life Plans, LCED/LOC, and proof of distribution uploaded to Precision Care.
- Ensure Day Hab Support Professional Specialists maintain case record documentation quarterly in accordance with HeartShare policies.
- Serve as a liaison with family members, guardians, and outside agencies as needed.
- Review and certify monthly summaries to ensure timely completion and reflection of progress.
- Review behavior plans, medical plans, and Staff Action Plans with DHSPS to determine if modifications or new goals are needed.
- Ensure timely collection of services information, including behavioral data and community inclusion calendars.
- Maintain program compliance with agency, state, and federal regulations.
- Complete all written reports and documentation on time.
- Ensure program participants are clean, safe, and provided with an appropriate environment.
- Train staff in following the Level of Service (LOS) for each person and ensure adequate coverage for each group.
- Attend all intake and admissions meetings; write initial Staff Action Plans prior to admission.
- Ensure Day Habilitation Support Professional Specialists update Staff Action Plans semi-annually and annually with input from participants, family, and team members.
- Report all incidents, including allegations of abuse and neglect, to the Day Hab Director or Assistant Director immediately and ensure the Justice Center Hotline is contacted as appropriate.
- Assist in filing 147 incident reports as necessary.
- Serve as an advocate for program participants.
- Provide in-service training for staff, including training in writing and implementing goals.
- Participate in in-service training, on-site workshops, and external training for professional growth.
- Maintain certification in CPR/First Aid and SCIP as required.
- Provide coverage for other supervisors as assigned.
- Secure documents for submission, including scripts, informed consent forms, and PPT sign-offs.
- Review Precision Care daily to ensure staff document services as needed.
- Create and submit monthly community inclusion calendars for all groups.
- Submit monthly reports to QA staff and upload them to Precision Care.
- Participate in interviewing, hiring, and initial in-service orientation of Day Habilitation Support Professional Specialists.
- Provide ongoing and timely supervision of assigned staff, including monthly supervision, quarterly observations, and annual evaluations.
- Complete probationary evaluations for new Day Service Support Professional Specialists within the identified timeframe.
- Approve staff evaluations by DHSPS.
- Undertake additional tasks and initiatives as assigned by the Day Hab Site Director or Assistant Director.
Requirements
- High School Diploma required; bachelor’s degree from a four-year college or equivalent preferred.
- Minimum one-year experience working with individuals with developmental disabilities.
- Valid NYS driver’s license preferred but not required.
