About the role
The Day Hab Program Activity Assistant (Instructor) plays a crucial role in implementing clinical and individual programs. They assist in the daily implementation of the Day Habilitation Services Plan (DHSP), daily schedules of activities, and recommended plans. The position involves providing support to persons served in a day habilitation setting, assisting in case management responsibilities, and participating in interdisciplinary team reviews.
Responsibilities
- Provides instruction and skill building in areas such as Self Help, Sensorimotor, Communication, Social Development, Independent Living, Behavior Development, Affective Development, and Wellness Development.
- Aids in case management responsibilities, as directed.
- Contributes to interdisciplinary team reviews and assists in developing the Day Habilitation Services Plan (DHSP).
- Maintains contact with family members, funding sources, and other staff members, including members of the Interdisciplinary team.
- Implements clinical and daily plans for persons served and ensures continuous improvement of programs.
- Maintains accurate documentation and completes progress notes according to guidelines.
- Works directly with persons served individually and in groups, aiming to develop their skills and ensure they are actively engaged in meaningful activities.
- Communicates with management regarding treatment and programs for persons served and participates in various teams, such as the Case Management Team.
- Supports members in crisis management using Positive Behavior Support (PBS) and Safety Care.
- Assists with the training of staff, as needed.
- Maintains the physical upkeep of the program area, monitors supplies and equipment, and ensures they are used, cleaned, and stored appropriately.
- Notifies managers of any needs identified and works to resolve them.
- Acts as an advocate for persons served in all areas.
- Fulfills all policies and procedures specific to the program and Vinfen.
- Works towards community integration of persons served through various opportunities and supports.
Qualifications
- Typical Requirements: One year of relevant work experience/training.
- Preferred /Required Education: A high school diploma or equivalent.
- Driving Requirements: Driving is a requirement for this position using either a Vinfen van or personal vehicle. Adequate insurance and a safe driving record are necessary.
- Physical Effort: Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, maneuver through narrow spaces or hallways, lift at least 25 pounds, and operate office equipment.
