About the role
The Day Hab Program Activity Assistant (Instructor) supports the implementation of clinical and individual programs, providing instruction and skill-building in various domains and assisting in case management.
Responsibilities
- Provides instruction and skill building in Self Help, Sensorimotor, Communication, Social Development, Independent living, Behavior Development, Affective Development, and Wellness Development.
- Aids in case management responsibilities, as directed.
- Contributes to interdisciplinary team reviews and assists in developing Day Habilitation Programs (DHSP).
- Maintains contact with family members, funding sources, and other staff members, including members of the Interdisciplinary team.
- Implements clinical and daily plans, ensuring continuous improvement of programs.
- Maintains documentation and completes progress notes according to guidelines.
- Works with individuals and groups, developing skills and ensuring engagement in meaningful activities.
- Communicates with management regarding treatment and programs for persons served.
- Participates in various teams, such as Case Management Team, and supports crisis management using Positive Behavior Support (PBS) and Safety Care.
- Trains staff as needed and maintains physical upkeep of the program area.
- Maintains supplies and equipment, ensuring they are used, cleaned, and stored appropriately.
- Notifies managers of needs and resolves issues.
- Acts as an advocate for persons served in all areas.
- Fulfills policies and procedures specific to the program and Vinfen.
- Promotes community integration through various opportunities and friendships.
Qualifications
- One year of relevant work experience/training.
- A high school diploma or equivalent.
- Valid driver's license and adequate insurance.
- At least 21 years of age.
- Ability to lift at least 25 pounds and operate office equipment.
- Specific vision abilities required for the job.
- CPR and First Aid certifications within 2 weeks of hire.
- New Employee Training (NET) and Safety Care certifications within 90 days of hire.
