About the role
Vinfen is a nonprofit organization providing community-based services to individuals with various disabilities. This position is within the Day Program, supporting individuals in day programs, implementing clinical and daily plans, and assisting with case management.
Responsibilities
- Provides instruction and skill building in areas such as Self Help, Sensorimotor, Communication, Social Development, Independent living, Behavior Development, Affective Development, Vocational Development, and Wellness Development.
- Aids in case management responsibilities, as directed.
- Participates in interdisciplinary team reviews and assists in developing Individual Service Plans (ISPs).
- Maintains contact with family members, funding sources, and other staff members.
- Ensures continuous improvement of programs through recommendations or implementing changes.
- Maintains documentation and completes progress notes according to guidelines.
- Works directly with individuals, in groups, and supports their development of skills.
- Communicates with management regarding treatment and programs.
- Supports crisis management using Positive Behavior Support (PBS) and Safety Care.
- Trains staff as needed.
- Maintains physical upkeep of the program area and monitors supplies and equipment.
- Notifies managers of needs and resolves issues.
- Acts as an advocate for individuals served.
- Follows policies and procedures specific to the program and Vinfen.
- Engages in community integration activities and develops friendships.
Qualifications
- One year of relevant work experience/training.
- A high school diploma or equivalent.
- Valid driver's license and ability to pass a driver's screening background check.
- Ability to lift at least 25 pounds and operate a computer and other office equipment.
- Physical ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and perform tasks of daily living.
- Specific vision abilities including close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
- Ability to assist with routine living activities such as cleaning, meal preparation, vacuuming, shoveling, and grocery shopping.
