Responsibilities
- Assist participants with activities of daily living (ADLs) such as bathing, dressing, grooming, hair care, mouth care, eating, and toileting following the care plan.
- Document care provided in accordance with established policies, guidelines, and applicable regulations; carry out, report, and document care given in an effective, timely manner as observed by the RN and through periodic record reviews.
- Assist with restorative and supportive care including ambulating, positioning, transferring, skin care, catheters, oxygen administration, and other aides as assigned.
- Monitor participant health status, including vital signs, weights, skin condition, appetite, and changes in functional ability, and report pertinent information to the clinical supervisor in a timely manner.
- Collaborate with multidisciplinary teams in care planning for participants.
- Monitor/assist participants with meals and help with feeding as necessary.
- Report any variance, accident, or unusual occurrence to appropriate personnel.
- Assist with loading and unloading of participants from vehicles and with participants' take-home supplies for the transportation department.
- Serve as a bus attendant on company vehicles, supporting participants while in transit and accompanying them in and out of facilities.
- Create and participate in group and one-on-one activities in accordance with the daily activity calendar when needed.
- Attend in-service and continuing education programs as scheduled.
- Perform other duties as assigned.
Required Qualifications
- Active and valid state-issued Certified Nursing Assistant (CNA) license.
- Current CPR certification (American Heart Association and/or American Red Cross) or ability to obtain.
- High School Diploma or GED equivalent.
Preferred Qualifications
- Experience in a care setting for frail and elderly patients participating in a group environment.
- Bilingual.
