Day Center CNA

Day Center CNA at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care.

Responsibilities

• Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting.
• Document care provided in accordance with established policies, guidelines, and applicable regulations.
• Carry out, report, and document care given in an effective, timely manner as observed by the RN and through periodic record reviews.
• Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen, and provides other aides as assigned or needed.
• Monitor participants' health status such as vital signs, weights, skin condition, appetite, and changes in functional ability; report pertinent information to clinical supervisor in a timely manner.
• Collaborate with multidisciplinary teams in care planning for participants.
• Monitor appetite and look for changes in functional ability and skin condition.
• Monitor/assist participants in a courteous manner with meals and help with feeding, as necessary.
• Report any variance, accident, or unusual occurrence to the appropriate personnel.
• Assist with loading and unloading of participants from vehicles.
• Assist the transportation department with participants’ take-home supplies.
• Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments; support participants while in transit and accompany participants in and out of facilities.
• Create and participate in group(s) and one-on-one activities in accordance with daily activity calendar when needed.
• Attend in-service and continuing education programs as scheduled and necessary.
• Other duties as assigned.

Requirements

• Active and valid state-issued Certified Nursing Assistant (CNA) license.
• Current CPR certification or the ability to obtain CPR certification (acceptable vendors: American Heart Association and/or American Red Cross).
• High School Diploma or GED equivalent.

Qualifications

• Experience in a care setting for frail and elderly patients participating in a group environment.
• Bi-lingual.

Pay

Posted pay range: $19.76 - $22.74 per hour.
