Day Center CNA at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care.
Responsibilities
- Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting.
- Document care provided in accordance with established policies, guidelines, and applicable regulations.
- Carry out, report, and document care given in an effective, timely manner as observed by the RN and through periodic record reviews.
- Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen, and provides other aides as assigned or needed.
- Monitor participants' health status such as vital signs, weights, skin condition, appetite, and changes in functional ability; report pertinent information to clinical supervisor in a timely manner.
- Collaborate with multidisciplinary teams in care planning for participants.
- Monitor appetite and look for changes in functional ability and skin condition.
- Monitor/assist participants in a courteous manner with meals and help with feeding, as necessary.
- Report any variance, accident, or unusual occurrence to the appropriate personnel.
- Assist with loading and unloading of participants from vehicles.
- Assist the transportation department with participants’ take-home supplies.
- Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments; support participants while in transit and accompany participants in and out of facilities.
- Create and participate in group(s) and one-on-one activities in accordance with daily activity calendar when needed.
- Attend in-service and continuing education programs as scheduled and necessary.
- Other duties as assigned.
Requirements
- Active and valid state-issued Certified Nursing Assistant (CNA) license.
- Current CPR certification or the ability to obtain CPR certification (acceptable vendors: American Heart Association and/or American Red Cross).
- High School Diploma or GED equivalent.
Qualifications

- Experience in a care setting for frail and elderly patients participating in a group environment.

Pay
Posted pay range: $19.76 - $22.74 per hour.
