Primary functions

The Registered Nurse's primary functions include administering skilled nursing care for clients in their place of residence, coordinating care with the interdisciplinary team, patient/family, and referring agency, and assuming responsibility for coordination of care.

Essential Functions

  • Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
  • Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
  • Obtain data on physical, psychological, social, spiritual, and cultural factors that may influence patient/family health status and incorporate that data into the POC.
  • Maintain up-to-date patient records so that all visits, contacts, problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
  • Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).
  • Education patients and/or their families regarding care, diet, medications, home safety, etc., using appropriate teaching methods.
  • Serve as a liaison between patients/families, team members, administration, physicians, and other healthcare professionals.
  • Attend interdisciplinary team meetings and other meetings as necessary and appropriate.
  • Perform on-call responsibilities and provide on-call services to patients/families as assigned.
  • Communicate effectively and provide clinical directions to Home Health Aide (HHA)/Skilled Nursing Assistant (SNA)/Licensed Vocational Nurse (LVN) to ensure quality and continuity of service provided.
  • Assess and reassess pain, utilize appropriate pain management techniques, and educate patients and families regarding pain management.
  • Arrange referrals to appropriate agencies when needed.
  • Monitor assigned cases to ensure compliance with requirements of third-party payers.

Knowledge/Skills/Abilities

  • Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
  • Ability to communicate effectively with patients and their family members and at all levels of the organization.
  • Knowledge of medications and their correct administration.
  • Competence in various nursing skills such as wound care, phlebotomy, and bowel/bladder training.
  • Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
  • Maintain current licensure certifications and meet mandatory continuing education requirements.
  • Commitment, professional growth, and competency.
  • Ability to work flexible schedules and travel locally.
  • Ability to maintain confidentiality.
  • Demonstrated ability to work independently and cooperatively as an effective team member.

Skills

  • Homecare Homebase Software Experience - Required.
  • OASIS Experience - Required.
  • Current state license as a Registered Nurse - Required.
  • CHPN certification - Preferred.
  • Proof of current CPR certification - Required.
  • Current driver's license - Required.
  • Must have reliable transportation and insurance - Required.
  • 1-2 years experience as a Registered Nurse in a clinical care setting - Required.
  • 2+ years Home health experience - Required.

Education

  • Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required.
  • Bachelor's of Science in Nursing - Preferred.

Pay Rate Range

55 - 59 USD hourly

The individual selected for this role will be offered the role as an employee of Atrium; compensation, medical benefits, fringe benefits, and other terms and conditions of employment shall be presented by Atrium upon offer. The pay rate range provided is a reasonable estimate of the anticipated compensation range for this job at the time of posting. The actual pay rate will be based on a number of factors, including skills, competencies, experience, location, and/or being pursued and other job-related factors permitted by law. In addition, this role will be eligible for overtime pay, in accordance with federal and state requirements.
