Primary functions
The Primary functions of the Registered Nurse are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency and assume the responsibility for coordination of care.
Essential Functions
- Assess home health and/or hospice patients in stable and emergent situations to identify the physical, psychosocial and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations.
- Implement, develop, document and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual and cultural factors that may influence patient/family health status and incorporate that data in to the Plan of Care (POC).
- Assure continuity of quality patient care delivered with appropriate documentation.
- Maintain up-to-date patient records so that all visits, contacts, problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
- Provide care utilizing infection control measures that protect both the staff and the Patient (OSHA).
- Edicates patients and/or their families regarding care, diet, medications, home safety, etc. Uses teaching methods appropriate to the age, condition, culture and assessed learning-readiness of the patient and/or family members. Consistently teaches and promotes self-care practices.
- Supervise and provide clinical directions to the HHA/STNA/LVN to ensure quality and continuity of service provided.
- Assess and reassess pain. Utilize appropriate pain management techniques. Educate the patient and family regarding pain management.
- Arranges referrals to appropriate agencies when need exists.
- Monitor assigned cases to ensure compliance with requirements of third party payer.
- Performs on-call responsibilities and provide on-call services to patients/families as assigned.
- Provides effective communication and serves as a liaison to patients/family, team members, administration, physicians and other health care professionals.
- Attends interdisciplinary team meetings and other meetings when necessary and appropriate.
Knowledge/Skills/Abilities
- Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
- Ability to communicate effectively with patients and their family members and at all levels of the organization.
- Knowledge of medications and their correct administration.
- Competent in performing various nursing procedures such as venipuncture, IV care, wound care, catheter care, and more.
- Competent in providing care and maintaining equipment used in home care.
- Ability to organize tasks, develop action plans, set priorities and function under stressful situations.
- Maintains current licensure certifications and meets mandatory continuing education requirements.
- Demonstrated ability to work independent and cooperatively as an effective team member.
Experience
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- Bachelor of Science in Nursing preferred.
- 1-2 years experience as a Registered Nurse in a clinical care setting.
- 2+ years Home health experience.
Education
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- Bachelor of Science in Nursing preferred.
Skills
- Homecare Homebase Software Experience.
- OASIS Experience.
- Current state license as a Registered Nurse.
- CHPN certification preferred.
- Proof of current CPR certification.
- Current driver's license.
- Reliable transportation and insurance.
Pay Rate Range
- 49 - 54 USD hourly.

- The individual selected for this role will be offered the role as an employee of Atrium; compensation, medical benefits, fringe benefits and other terms and conditions of employment shall be presented by Atrium upon offer.
- The pay rate range provided is a reasonable estimate of the anticipated compensation range for this job at the time of posting. The actual pay rate will be based on a number of factors, including skills, competencies, experience, location and/or being pursued and other job-related factors permitted by law.
- In addition, this role will be eligible for overtime pay, in accordance with federal and state requirements.
