Job Summary
TheraCare Home Health is seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. In this role, you will manage a caseload of patients, complete comprehensive assessments, and coordinate care across the interdisciplinary team. You will play a key role in supporting patients throughout their home health episode while promoting independence, safety, and positive clinical outcomes.
Qualifications
- Graduate of an accredited Diploma, Associate, or Bachelor’s degree in Nursing
- Current Registered Nurse (RN) license in the state of Texas
- Minimum 2 years of RN clinical experience; home health experience preferred
- Strong knowledge of home health regulations, OASIS documentation, and care planning
- Experience completing Start of Care (SOC), Recertifications, and Resumption of Care visits preferred
- Able to manage a patient caseload and work independently in a field-based environment
- Experience coordinating care with interdisciplinary teams (PT, OT, ST, MSW, HHA)
- Strong clinical assessment, documentation, and critical thinking skills
- Excellent communication and interpersonal skills with patients, families, and physicians
- Reliable transportation with valid driver’s license and auto insurance
- Proficiency with EMR systems (HomeCare HomeBase preferred)
Essential Functions
- Manage and coordinate all aspects of home health patient care and case management
- Perform Start of Care (SOC), Recertification, and Resumption of Care assessments
- Develop, implement, and update individualized plans of care based on patient needs
- Career with interdisciplinary team members to ensure continuity of care
- Supervise and provide clinical direction to LVNs and Home Health Aides
- Lead and participate in interdisciplinary case conferences (IDG/IDT)
- Monitor patient progress and adjust care plans to achieve desired outcomes
- Provide patient and caregiver education on disease management, medications, and home safety
- Complete accurate and timely clinical documentation in compliance with regulatory standards
- Career discharge planning and ensure smooth transitions of care
- Monitor visit utilization, productivity, and patient outcomes
- Maintain compliance with agency policies, procedures, and state and federal regulations
