About the role

Atrium is seeking a temporary contractor to work at CenterWell as a Registered Nurse. The primary functions include administering skilled nursing care, coordinating care with the interdisciplinary team, and assuming responsibility for coordination of care.

Responsibilities

  • Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
  • Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
  • Obtain data on physical, psychological, social, spiritual, and cultural factors that may influence patient/family health status and incorporate that data into the POC.
  • Maintain up-to-date patient records ensuring all visits, contacts, problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
  • Provide care utilizing infection control measures that protect both the staff and the patient.
  • Teach patients and/or their families about care, diet, medications, home safety, etc., using appropriate teaching methods.
  • Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality and continuity of service provided.
  • Assess and reassess pain and utilize appropriate pain management techniques. Educate patients and families about pain management.
  • Arrange referrals to appropriate agencies when needed.
  • Monitor assigned cases to ensure compliance with requirements of third-party payers.
  • Perform on-call responsibilities and provide on-call services to patients/families as assigned.
  • Communicate effectively with patients and their families and other healthcare professionals.
  • Attend interdisciplinary team meetings and other meetings as necessary.
  • Perform other related duties as assigned or requested.

Requirements

  • Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
  • Effective communication skills with patients and their family members and at all levels of the organization.
  • Knowledge of medications and their correct administration.
  • Competence in performing various nursing skills such as wound care, phlebotomy, and bowel/bladder training.
  • Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
  • Maintain current licensure certifications and meet mandatory continuing education requirements.
  • Commitment to professional growth and competency.
  • Ability to work flexible schedules and travel locally.
  • Ability to maintain confidentiality.
  • Commitment to working independently and cooperatively as an effective team member.

Skills

  • Homecare Homebase Software Experience
  • OASIS Experience
  • Current state license as a Registered Nurse
  • CHPN certification (preferred)
  • Proof of current CPR certification
  • Current driver's license
  • Reliable transportation and insurance
  • One to two years experience as a Registered Nurse in a clinical care setting
  • Two plus years home health experience

Qualifications

  • Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
  • Bachelor's of Science in Nursing (preferred)
