Position Summary We are looking for a passionate and dedicated Cultural Community Health Worker (CCHW) to support and uplift Black, Indigenous, and People of Color (BIPOC) communities. As a trusted connector, you’ll provide culturally responsive outreach, health education, and care navigation, while addressing social determinants of health and advocating for individual and community needs. You’ll meet people where they are—in their homes, neighborhoods, hospitals, and community spaces—to help reduce health disparities, increase access to services, and promote human rights and social justice. What You’ll Do Provide health education and connect individuals to resourcesProvides assistance in the operation of the One World Pantry. This can include, but not limited to, driving to pick up products, unloading and storing food donations and deliveries, ensuring food pantry, office space, storage room, etc. are all clean and well organized and interacting with guests to the department, assessing for nutrition needs, healthcare needs, resource needs and available financial support for healthcare.Help people apply for health benefits and access needed careServe as a trusted advocate for underserved communitiesCollaborate with families and partners to address complex social needs Minimum Requirements High school diploma or GED1+ year CHW or related community experienceComfortable visiting homes, hospitals, and community settingsStrong empathy, communication, and collaboration skillsProficient in Microsoft OfficeBilingual (English/Spanish, Arabic, Somali, or Pashto) preferred Why This Role Matters You’ll be a frontline advocate for equity and wellnessYou’ll connect people to life-changing resourcesYou’ll be part of a team committed to justice and health for all
