Responsibilities
- Provides daily direction to Direct Support Staff
- Fulfills written plan of care for each member in the program
- Provides hands-on direct support to members of the CSL program
- Collaborates and communicates with CSL Manager, Case Managers, and natural supports to compliment member’s plan of care
- Maintains workload of the group and re-assigns tasks as needed
- Completes necessary documentation for each member served
- Trains staff in conjunction with the CSL Manager
- Buils and maintains relationships with members served, funders, and community stakeholders
- Participates in On-Call rotation responsibilities
- Applies GT Independence values during all workplace practices
- Performs other duties as assigned
Qualifications
- 2 years of experience relevant to the work performed
- Knowledge base in supervision, case management, and leadership
- Excellent organizational and problem-solving skills
- Ability to consistently meet deadlines
- Team-work oriented
- Strong ability to maintain confidentiality
