Crisis & Triage Management Specialist

Job Summary Provides ambulatory behavioral health services including evaluation, diagnosis, treatment planning, case management, and psychotherapeutic interventions for individuals, families, and groups, including Intensive Outpatient Programs. Responsibilities Develops, implements, and evaluates social work programs and policies.Works in a team to provide telephonic triage, assessment, crisis management, and follow-up for members requesting behavioral health services.Coordinates admissions, discharges, and referrals for psychiatric care.Performs clinical diagnosis, risk assessments, crisis interventions, and provides psychotherapy using evidence-based modalities.Evaluates patient mental and psychological status, determines crisis or risk levels, and refers to appropriate care.Assesses treatment needs, manages case plans, and educates members on available services.Participates in group programs, evaluates treatment outcomes, and maintains clinical records.Collaborates with healthcare team members, reviews complex cases, and ensures adherence to clinical guidelines.Maintains clinical expertise, stays updated on methods and interventions, and contributes to quality assurance activities.Provides after-hours assessments in emergency settings as needed and performs other clinical duties. Qualifications Master’s degree in social work from an accredited institution and supervised clinical training.Licensed Clinical Social Worker (Hawaii) and a National Provider Identifier.Basic Life Support certification.Knowledge of DSM assessments, diagnostic evaluations, and evidence-based treatment approaches.Strong interpersonal, decision-making, and communication skills, with the ability to handle stressful situations over the phone. Preferred Skills Experience with documentation standards, primary care behavioral health, addiction treatment, emergency department consultation, and outpatient or inpatient mental health services.
