Additional Information About The Role Full time Benefit Eligible Location of support – Farmington, MO Monday through Friday from 8:30 am to 5 pm No weekends, holidays or on call Responding to crisis situations and stabilizing the situation Working with a variety of individuals – youth, adult, etc. No caseload Supporting individuals presenting in crisis, and Provide clinically appropriate crisis intervention and risk assessments through appropriate platforms: phone, Teladoc, face-to-face interactions. Overview BJC Behavioral Health is a community health center that provides and coordinates behavioral health services for more than 8,000 seriously mentally ill adults and seriously emotionally disturbed children in St. Louis City, St. Louis County, St. François, Iron and Washington counties. As an Administrative Agent of the Missouri Department of Mental Health (DMH), BJC Behavioral Health serves as a major point of entry for people eligible for mental health services funded by DMH and is responsible for serving as gatekeeper to the public mental health system. Inform, outreach, screen for services, and successfully refer people to the appropriate services within the organization and larger community in a manner that is timely and respectful. Preferred Qualifications Role Purpose The Clinical Case Manager is responsible for providing case management services to maximize opportunities available to people living in the community who are trying to recover from the catastrophic effects of a serious mental illness. Responsibilities Authorizes, coordinates, monitors, and adjusts services with contract providers.Identifies, develops, and maintains good relationships and linkages with a wide range of community resources.Ensures follow through with appointments by assisting clients to keep appointments.Provides collaborative clinical case management services.
