About the role
The CRIS APS Caseworker investigates reports of abuse, neglect and exploitation in Vermilion County involving adults with disabilities 18 and older and adults 60+. They assist individuals who may need services, communicate with various stakeholders, assess potential clients, provide resources and referrals, and advocate for clients.
Responsibilities
- Complete assessments of potential clients to determine need for services.
- Create intake, case notes, and case plans based on individual needs.
- Maintain accurate, complete, and timely documentation on all client activities and services provided.
- Make appropriate referrals to other agencies and programs administered by CRIS to meet the needs of clients.
- Communicate clearly, both orally and in writing, with older individuals and disabled individuals, as well as program employees, community organizations, professionals, and community members.
- Organize and prioritize caseload to meet required standards of the Adult Protective Services standards set by the Older American’s Act, Adult Protective Services Act, and IDOA.
- Advocate/support each client based on individual needs.
- Perform face-to-face assessments of alleged victims and abusers.
- Determine victim/caregiver needs and provide appropriate intervention services.
- Provide follow-up contact and outreach services to ensure clients stay active and engaged.
- Maintain a network with the client's physician, family members, judicial system, state's attorney's office, women's shelter, sexual assault advocates, law enforcement, attorneys, etc., to ensure services are provided.
- Become familiar with community services, resources, and programs to assist clients.
- Prepare intakes and review case files accurately and timely, as assigned.
- Work closely with other CRIS administrative and program staff to accomplish program and agency goals.
- Complete monthly and quarterly reports as stipulated in grant requirements.
- Solicit and identify individuals aged 60 and over, as well as those aged 18-59 with disabilities in Vermilion County who may need services.
- Act as a mandated reporter when necessary.
Qualifications
- Certifications: Driver's License, LPN, RN.
- Education: Nursing diploma, Bachelor's degree in Human Services, Healthcare, Criminal Justice, or Social Work.
- Work Experience: Healthcare in a related field.
Skills
- Clear communication skills, both oral and written.
- Ability to perform face-to-face assessments and advocate for clients.
- Knowledge of appropriate intervention services and the ability to communicate the need to petition the court.
- Familiarity with community services and resources.
- Dependable transportation and ability to travel to client homes.
