Counselor / Social Worker - Senior Care Unit
About the role
Brentwood Hospital is seeking a dynamic Counselor / Social Worker to join their team. The ideal candidate will provide quality case management and psychotherapeutic services to various patient populations, including adults, adolescents, and children, along with their families.
Responsibilities
- Conduct psychosocial assessments to document history and identify preliminary issues for treatment focus.
- Provide group and family process-oriented therapies, as well as conduct educational and other didactic groups for patients and family members.
- Summarize daily progress notes detailing the patient’s progress toward treatment goals.
- Record contact with family, significant others, and community agencies in progress notes.
- Develop and coordinate an individualized discharge plan for patients, utilizing treatment team and chart information to determine aftercare needs.
- Identify and assess family or community resources such as group homes or mental health practitioners to meet patients’ aftercare needs.
Qualifications
- Master's Degree in social work, counseling, or related field.
- Experience in inpatient or outpatient mental health settings preferred.
- LPC/PLPC or LCSW/LMSW license required.

Pay
Compensation is commensurate with experience.
