What You Will Do
- Provide assessment, diagnosis, and psychotherapy for individuals, couples, families, and groups
- Support residents and families with concerns including relationships, grief, depression, financial stress, medical conditions, and life transitions
- Develop individualized care plans and maintain accurate clinical documentation, progress notes, and referrals
- Facilitate or develop support groups addressing psychosocial and emotional needs
- Collaborate with physicians, social workers, clergy, families, and external providers to coordinate care
- Making appropriate referrals for treatment, medication management, or community resources
- Participate in community outreach and educational presentations on mental and behavioral health
- Evaluate program effectiveness through resident feedback and clinical outcomes
- Support additional duties as assigned within scope of qualifications
Case Management & Home Care Collaboration
- Cookordinate resident care across the continuum of services at La Posada
- Build relationships with residents to proactively identify emotional and psychosocial needs
- Meet with new residents within 30 days of move-in for initial assessment and ongoing support
- Assist residents and families through transitions across levels of care, providing education and emotional support
What We’re Looking For
- Master’s degree in Social Work required, with strong clinical assessment and intervention skills
- LMSW or LCSW preferred
- Minimum of 3 years of experience in social work or counseling with adults, ideally older adults
- Strong interpersonal, communication, and documentation skills
- A compassionate, people-centered approach with emotional sensitivity and sound judgment
- Ability to work independently and collaboratively in a team environment
