COTA - Certified Occupational Therapist Assistant

Elderwood at Hornell is seeking a Certified Occupational Therapy Assistant (COTA) to join our skilled nursing and rehabilitation team in Hornell, NY. This is a full-time opportunity in a supportive, team-driven environment focused on high-quality patient care.

About the role

• Provide occupational therapy treatments under the direction of a licensed Occupational Therapist (OT)
• Assist patients with activities of daily living (ADLs), mobility, and rehabilitation goals
• Conduct individual and group therapy sessions
• Accurately document patient progress and treatment outcomes in the EMR system
• Collaborate with therapy, nursing, and interdisciplinary teams
• Help patients improve independence, strength, and overall quality of life

Treat residents recovering from stroke, orthopedic surgery, joint replacements, neurological conditions, general medical decline, and long-term care needs.

Requirements

• Current New York State COTA license required
• Experience in skilled nursing, rehab, or long-term care preferred
• Strong communication and teamwork skills
• Passion for working with geriatric/elderly populations
• Ability to read, write, speak, and understand English at an intermediate or advanced level
