About the role

This LPN position is in a Residential home setting, providing care to a small group (2-5) of individuals. The opportunity offers the reward of enhancing the lives of individuals with disabilities by delivering quality home care and making a meaningful difference.

The Residential Licensed Practical Nurse (LPN) reports to the Nurse Manager for nursing-related duties and the Residential House Manager (RHM) for programmatic duties. The LPN will attend medical appointments for the individuals supported, provide nursing care during on-duty hours, establish plans of care, and facilitate health education and training for direct care professionals to ensure health and safety.

Schedule

  • Sunday: 7:00 AM – 3:00 PM
  • Thursday: 11:00 PM – 9:00 AM
  • Friday: 11:00 PM – 7:00 AM
  • Saturday: 11:00 PM – 7:00 AM

Total hours: 34 per week

Pay

New pay rate: $31.00 per hour

Sign-on bonus: $1,500

Responsibilities

  • Conduct assessments specific to each person’s health care needs.
  • Assist in creating a Problem/Needs list for each person in the residence with the nursing team.
  • Review the nursing Problem/Needs list bi-monthly, after each hospitalization, or as needed.
  • Administer IV therapy as needed via PICC line (pre-mixed medication in IV solution only; no IV push medication). PICC training required prior to administration.
  • Administer medications as assigned.
  • Assist with Health Care Provider visits.
  • Write progress notes and weekly nursing assessments.
  • Notify the Health Care Provider of any medical changes or concerns, then communicate and document all information.
  • Communicate any discharge instructions, treatments, or medication changes to the RHM and all staff.
  • Work with Home Health nursing if applicable.
  • Physically assist individuals in transfers and carries, both ambulatory and non-ambulatory (individuals may weigh 150–250 pounds).
  • Assist persons in bathing and personal hygiene tasks, which may include lifts and physical support.
  • Educate and assist staff and individuals with meal preparation, nutrition, and dietary needs.
  • Follow agency policies and procedures as defined in the agency’s Policy and Procedure Manual.
  • Identify safety risks and possible alternatives, and advocate for the rights of all individuals.
  • Maintain confidentiality in keeping with HIPAA regulations.
  • Handle related job duties in compliance with the agency’s Programmatic Policy Manual.
  • Ensure the program maintains standards outlined in the Department of Developmental Services mission statement, the Survey and Certification Tool, and Performance-Based Objectives.
  • Attend and/or participate in training of residential staff during staff meetings.
  • Attend nursing team meetings.
  • Implement the agency mission through service delivery, staff development, and resource acquisition.
  • Provide recommendations to your supervisor regarding ways to improve current systems.
  • Maintain CPR certification.
  • Maintain Safety Care certification, if required by position/program.
  • Other duties as assigned.

Requirements

  • Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
  • At least one year of full-time working experience as an LPN.
  • Must be 21+ years old.
  • Valid driver’s license and daily use of a car on-site.
  • Available by telephone for daily contact.

  • Ability to complete Agency Nursing Orientation.
  • Positive attitude and ability to establish rapport with individuals with disabilities, their families, co-workers, and service providers.
  • Organized, dependable, and even-tempered with a professional phone manner.
  • Effective verbal and written communication skills.
  • Strong interpersonal and communication skills, with the ability to work with a wide range of employees.
  • Must be able to bend, lift, stoop, reach, and file.
