Coordinator Patient Care - C1 Rehab - Nights

About the role

C1 Rehabilitation is a 24 bed post-acute rehab unit that provides care to patients with conditions due to stroke, brain injury, arthritis, amputation. Through the collaboration with the Mary Free Bed Rehabilitation Network, northern Michigan patients have increased access to specialists, streamlined care coordination and the highest standard of rehabilitation care close to home.

Responsibilities

• Collaborate with interdisciplinary teams to develop and implement patient care plans
• Provide direct patient care including assessments, treatments, and interventions
• Document patient progress and communicate effectively with patients, families, and other healthcare providers
• Ensure compliance with regulatory standards and policies

Requirements

• Active Registered Nurse license in the State of Michigan
• Bachelor of Science in Nursing (BSN) degree within 5 years of start date
• Minimum of 2 years of relevant experience, 2 years in the specialty area preferred
