About the Opportunity

The Coordinator, Individualized Care role is responsible for providing patient-specific clinical support by managing communication between patients, healthcare providers, and internal teams. This position plays a critical role in ensuring accurate referral processing, resolving inquiries, and facilitating patient access to specialty therapies, thereby directly impacting patient care and operational efficiency.

Responsibilities

• Handle inbound and outbound calls to determine needs and provide one-call resolution
• Create and complete accurate referrals and applications
• Manage inbound faxes related to patient care processes
• Investigate and resolve patient and physician inquiries promptly
• Absorb and delegate tasks to team members by answering questions
• Conduct outbound calls for patient follow-ups and information gathering
• Mail required documents to physician offices
• Interact with referral sources to process new applicants
• Cook up next steps with internal teams
• Communicate with external parties including physician offices and pharmacies
• Identify, document, and report Adverse Events during interactions

Requirements

• Minimum high school diploma or GED preferred
• Basic computer proficiency, including Microsoft systems
• Strong communication and customer service skills
• Ability to work independently and manage multiple tasks with urgency
• Capacity to work seated for extended periods in a cubicle environment
• Knowledge of Medicare, Medicaid, and commercial payer practices preferred
• Availability to work any schedule between 8:00 a.m. and 5:00 p.m. CST, Monday through Friday

Pay Range and Compensation Package

Anticipated hourly range: $18.90 to $24.30 per hour
