About the Role

The Intake Coordinator is responsible for the acceptance, processing, and coordination of all referrals for patient care services. This role determines eligibility and benefits for new referrals or readmissions, obtains authorizations, and secures financial obligations. The coordinator works closely with pharmacy, nursing, and client service specialists to initiate patient care services.

Responsibilities

  • Answers telephones using appropriate etiquette and communicates with referral sources, patients, family members, clinicians, and payors.
  • Initiates the intake process by gathering relevant data from referral sources required to admit patients to services.
  • Notifies departments of new referrals and coordinates information among referral sources, clinicians, and sales.
  • Provides appropriate documentation to clinical departments for initiation of services.
  • Determines patients' financial obligations, out-of-pocket expenses, and copays.
  • Contacts patients prior to the start of services to discuss financial obligations and arrange payments.
  • Completes start-of-care paperwork for patient signature, including admissions agreement, financial obligation, advanced beneficiary notice, and compassionate care application as required. Tracks the return of signed copies within the NuFACTOR system of record.
  • Enters complete referral information into the NuFACTOR system of record and logs all communications and updates in assessments and notes while admitting patients for service.
  • Complies with all NuFactor policies, procedures, and compliance requirements.
  • Adheres to company policies, federal and state regulations, and laws.
  • Displays dedication to position responsibilities and achieves assigned goals and objectives.
  • Represents the company in a professional manner and appearance.
  • Works effectively with coworkers, internal and external customers, and others by sharing ideas constructively and listening objectively.
  • Complies with the Injury and Illness Prevention Program by working safely and reporting hazards.
  • Ensures conduct aligns with all Compliance Program Policies and procedures, reporting any concerns or violations immediately.
  • Performs other duties as assigned.

Requirements

  • High School diploma or equivalent required.
  • One (1) year of healthcare-related experience in a pharmacy, home healthcare, infusion-related setting, or hospital is preferred.
  • Experience with benefit verification and prior authorizations for specialty medications is preferred.

    • Ability to work with a high degree of accuracy in an organized, detail-oriented manner.
    • Ability to communicate effectively with healthcare professionals and peers.
    • Ability to prioritize work and make decisions independently, often quickly in response to dynamic patient needs.
    • Working knowledge of state, federal, and Joint Commission guidelines/regulations/laws.
    • Ability to exercise discretion and maintain confidentiality to HIPAA standards.
    • Proficiency in Microsoft Office and internet technologies.

Physical Requirements

  • Vision, hearing, speech, and movements requiring the use of wrists, hands, and/or fingers.
  • Ability to view a computer screen for long periods and sit for extended periods.
  • Ability to lift and maneuver items of at least 20 lbs.
  • Ability to travel occasionally.
  • Normal office working conditions.

  • Ability to pay close attention to detail and work as a team member to ensure excellent customer service.
