About the role

The Coordinator oversees services for individuals in the program, ensuring compliance, quality care, and accurate documentation while supporting clients and care staff.

Responsibilities

• Submit assessments and documentation to the appropriate state health and human services department.
• Develop and maintain individual plans of care and habilitation plans.
• Ensure services align with approved individual plans.
• Monitor service providers for quality and compliance.
• Coordinate with case management agencies.
• Maintain service records and evaluation documents.
• Ensure timely billing and documentation.

Requirements

• High school diploma or GED.
• Minimum of one year of administrative or clerical experience (experience in home health preferred).
• Strong communication and organizational skills.
• Proficiency in Microsoft Word, Excel, Outlook, and web browsers.
• CPR certification (required; may be obtained upon hire).
• At least 18 years of age.
• Valid Texas driver’s license.
• Clear criminal background check.
• Ability to travel independently to client homes.
• Ability to lift up to 50 lbs. and assist with transfers (Hoyer lift, positioning, ambulation).
• Ability to sit or stand for extended periods and work on a computer.

Skills
