About the role
The role is responsible for managing patient care coordination and ensuring high-quality healthcare services are delivered efficiently.
Responsibilities
- Manage patient care coordination and ensure seamless transitions between healthcare providers.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Provide education and support to patients and families regarding their healthcare needs.
- Monitor patient progress and adjust care plans as necessary.
- Ensure compliance with healthcare policies and regulations.
Requirements
- Bachelor's degree in Healthcare Administration, Nursing, or related field.
- Minimum of 2 years of relevant work experience in healthcare settings.
- Strong communication and interpersonal skills.
- Proficiency in Microsoft Office Suite.
