Continuity of Care Navigator - QMHP
Job Summary
Continuity of Care Navigators perform moderately complex coordination work, provide preliminary assessment of a client's presenting psychiatric and/or co-occurring disorder(s), develop a plan to provide linkage to services sought or needed by the client, and coordinate appropriate services to facilitate each client's success in achieving and maintaining the maximum possible level of independent functioning. This position is community based and provides short term services to adult clients to provide linkage and advocacy in establishing care with Integral Care and partners such as Community Care and Central Health.
About the Role
This role has a primary focus on advocating to ensure client connection to ongoing care for any indicated needs. This position serves individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. The services that Continuity of Care Navigators provide require frequent coordination with external and internal providers as necessary to support individuals in achieving independent functioning.
Responsibilities
- Coordinate services to assure that the individual is connected to any indicated services.
- May include coordination to assist the client in obtaining or maintaining employment, housing, medical care, dental care, food, clothing, utilities, financial assistance, transportation, legal assistance, or any other basic human need.
- Provide bilingual communication services as required.
Requirements
- Current and valid driver's license and a clean driving record.
- Bilingual in Spanish (preferred).
Qualifications
- Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.
- Master's degree preferred.
Skills
- Ability to work independently and as part of a team.
- Strong communication skills, both verbal and written.
- Knowledge of trauma-informed care principles.
- Familiarity with community resources and service providers.

Pay
$24.18 - $25.39 an hour, based on education.
Schedule
Full-Time. Workdays: Monday - Friday. Workhours: 8:00 AM - 5:00 PM.
