Responsibilities

• Serve as a key member of our care team, focusing on the health and well-being of our older adult population through the 4Ms framework (Medication, Mentation, Mobility, and What Matters).
• Work with Winding Waters Public Health leadership and state collaborators to implement this structured program at Winding Waters, and track success through data collection and reporting.
• In-Home Care & Assessment: Conduct home visits to complete Connected Care Protocols, assess health and social needs, identify fall hazards, and assist patients with medication management and Advance Directives.
• Bridging & Coordination: Collaborate closely with primary care providers and health care teams to advocate for patients and align care plans with what matters most to them.
• Outreach & Education: Deliver culturally competent health education on healthy aging, guide families on available social services, and assist with application processes.
• Crisis Support & Advocacy: Offer emotional support, intervene during health-related challenges, and connect patients to mental health or crisis resources when needed.
• Closing the Loop: Maintain accurate records in our Electronic Health Record (EHR) system, track outcomes, and participate in continuous program evaluation.

Qualifications

• High school diploma or equivalent.
• Valid driver’s license, insured vehicle, and willingness to travel locally.
• 90-hour Community Health Worker Certification, or the ability to become certified.
• Minimum of two years of experience in a health or social service-related field preferred.
• Proficiency in basic data entry, record-keeping, and EHR systems.

Pay

Competitive wage range of $18.50–$30.00/hour, commensurate with experience and certified skill sets.
