About the role

The DHS Complex Case Service Coordinator will provide assessment, outreach, linkage, and assist with the acquisition and coordination of resources and services for clients on their caseload. This individual will administer the Child and Adolescent Needs and Strengths Assessment (CANS) to assist in service planning for youth and/or their parent/guardian. The coordinator will also provide ongoing support for youth placed in specialized group homes, including Devereux Mapleton, Pathways, Horizon, Keystone, Pathways Enhanced (Oil City), and Family Links shelter. These programs may change based on DHS-contracted providers.

The CANS assessment is a multi-purpose tool developed to support care planning, level-of-care decision-making, quality improvement initiatives, and outcome monitoring. The assessor will facilitate communication across child-serving agencies and identify the needs and strengths of youth and their families to support the youth’s path toward recovery. Work is performed with considerable initiative and independent judgment in accordance with established regulations, policies, and procedures.

Responsibilities

  • Maintain CANS credentialing on a yearly basis
  • Meet all timelines identified by the Department of Human Services screening process
  • Maintain a caseload of 25–30 youth
  • Complete case notes and documentation within the DHS documentation system
  • Complete the CANS assessment in the Key Information Demographic System (KIDS)
  • Implement the CANS screening tool for referred eligible youth placed in care
  • Attend and participate in County Integrated Children’s Planning meetings
  • Comply with and adhere to HIPAA confidentiality policy
  • Coordinate with families, providers, Office of Children Youth and Families, juvenile probation, educational system, and other community resources to facilitate appropriate services
  • Attend and participate in personal development to remain knowledgeable of current trends, initiatives, resources, and services available for clients and families

Requirements

  • Possess Act 33, Act 34, and FBI clearances
  • Valid Pennsylvania motor vehicle license and access to a personal vehicle
  • Current auto insurance meeting minimum collision coverage
  • Knowledge of guidelines and regulations of all levels of service coordination
  • Strong oral and written communication skills
  • Excellent leadership qualities
  • Ability to work independently

Qualifications

  • Master’s degree in Social Work, Education, or Psychology, or RN Degree, or Bachelor’s degree in Social Work, Education, or Psychology with two years of experience in the CASSP system

Pay

$24.52 per hour ($51,000 annually)
