Complex Care Team Clinical Social Worker Lead (LICSW)
About the Role
The Complex Care Team Clinical Social Worker is an integral member of the Complex Care Management Team, collaborating to reduce length of stay and readmission due to psychosocial factors. This role supports and interfaces with interdisciplinary healthcare teams to provide and oversee psychosocial care for patients with complex psychosocial risks (e.g., safety concerns, protective services, homelessness). The position focuses on maximizing emotional, social, and physical well-being while ensuring high-quality post-discharge care.
Responsibilities
- Conduct rapid clinical biopsychosocial assessments, including evaluation of social history, mental health, substance use, support systems, cultural issues, abuse/neglect/safety concerns, and intimate partner violence.
- Assess family dynamics, decision-makers, coping styles, resources, and cultural considerations to inform care plans.
- Formulate diagnostic impressions, recommendations, and care plans in collaboration with the interdisciplinary team to optimize patient outcomes and reduce readmissions.
- Manage consultation requests in a fast-paced environment, prioritizing needs based on patient acuity and barriers to safe discharge.
- Provide mandated assessments for suspected abuse (child, disabled adult, elder) and specialized safety assessments for violence; file mandated reports as required.
- Employ clinical interventions such as psychotherapy (individual, couples, families, group), short-term counseling, crisis intervention, safety planning, care coordination, and complementary therapies.
- Advocate for patients and families to access services and resources, ensuring equitable and efficient use of hospital and community resources.
- Collaborate with internal and external stakeholders, including Office of Patient Relations, Office of General Counsel, Child Protection Team, HAVEN, Police and Security, and community agencies.
- Identify psychosocial, financial, legal, psychiatric, or substance use barriers to discharge and facilitate safe transitions in care.
- Coordinate and facilitate family team meetings in collaboration with the interdisciplinary team.
- Provide psychosocial consultation to healthcare providers, social work staff, students, and the community on patient-related issues.
- Present cases at weekly Complex/Leadership meetings and participate in Departmental and Hospital committees.
- Complete timely and accurate clinical documentation, including psychosocial assessments, progress notes, and post-acute referrals.
- Adhere to professional clinical standards, Code of Ethics, and Departmental/Hospital policies and procedures.
- Participate in individual clinical supervision, continuing education, and professional development activities.
- Comply with licensure requirements and stay current with social work and healthcare developments pertinent to the role.
Qualifications
- Master’s Degree in Social Work (required; no experience in lieu of degree).
- Licensed Independent Clinical Social Worker (LICSW) state license.
- 5-7 years of clinical social work experience.

- Expertise in specialized clinical diagnoses and interventions, routinely sought for consultation on complex cases.
- Advanced clinical assessment, diagnostic, and intervention skills, with a focus on mentoring and training staff.
- Exceptional organizational skills and ability to prioritize in a fast-paced, dynamic environment.
- Strong crisis intervention, treatment, and communication skills (oral and written).
- Competence in assessing and intervening in abuse/neglect, violence, trauma, grief, loss, and bereavement.
- Ability to work effectively with diverse racial, ethnic, and socioeconomic backgrounds.
- High degree of flexibility, independence, and initiative to manage multiple acute concerns.
- Strong interpersonal skills for collaboration with interdisciplinary teams, patients, and families.
- Knowledge of medical and psychiatric illnesses, treatments, and medications (e.g., psychotropic, chemotherapy).
- Ability to navigate complex systems and advocate for patients/families.
- Competency in age-specific behaviors, cultural issues, and the impact of illness on patients/families.
- Leadership in driving psychosocial care plans and contributing to education, training, and clinical supervision.
- Proficiency in documenting in medical records according to Departmental policies and maintaining patient confidentiality (HIPAA).

Pay
$105,684.80 - $153,722.40 annually.
