About the role

Coordinate and provide services and resources to individuals/families necessary to promote rehabilitation, recovery, and resiliency. Identify strengths and barriers that could impede the development of skills needed for independent functioning in the community. Support individuals and families in crisis situations; provide personalized interventions to develop or enhance decision-making ability. Patient care assignment may include children at risk of or experiencing serious emotional, neurobiological, and/or behavioral disorders; adults with severe mental illness, individuals with chronic substance abuse, and/or individuals with co-occurring disorders. Ensure adherence to hospital and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult, and geriatric age groups.

Responsibilities

• Patient Centered MED: Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable
• Navigation/Patient Advocacy: Work to eliminate barriers to access to culturally competent medical services; assist patients to access community resources and financial assistance programs and services
• Collaboration: Collaborate with medical providers, multidisciplinary team members and community resource providers to assist patient in meeting treatment goals
• Life Skills: Facilitate development of skills in activities of daily living, interpersonal coping, socialization, and community functioning; adapt to home, school, work, recreational, and social environments; promote self-care, nutrition, and money management
• Development: Plan for professional growth based on self-assessment, evaluation and feedback; assume responsibility for acquiring knowledge and experiences to meet goals
• Wellness: Provide information and resources on mental health conditions; mentor to promote recovery and resiliency; teach symptom monitoring, management, and relapse prevention
• Medication Support: Provide information about illnesses and medications, side effects, and discuss concerns with providers; facilitate self-motivational skills for medication regimen and consequences to independent living
• Service Plans: Assist in development and coordination of individualized service plans including recovery/resiliency management, crisis management, and advanced directives concerning behavioral healthcare
• Resource Coordination: Assist clients in obtaining and coordinating rehabilitative, medical, and other services; help obtain financial and medical benefits/entitlements; assist in obtaining and maintaining safe, affordable, and stable housing; provide support and mentoring to behavioral health consumers involved in the judicial system
• Support Services: Encourage development and succession of natural supports in workplace, school, family, and community environments
• Assessment: Identify personal strengths, needs, and barriers to self-identified goals; conduct ongoing assessments to determine if accessed services meet the individual’s needs
• Crisis Intervention: Assess, support, and intervene in crisis situations; facilitate development and use of individual crisis management plans; identify alternatives to hospital emergency departments and inpatient services
• Tracking: Coordinate and monitor service use; track client activities related to care plans such as appointment attendance; review documentation of in-house and external providers; conduct post-discharge follow-up as appropriate
• Documentation: Document all client encounters and those made on behalf of clients; complete and submit billing documentation; maintain current and comprehensive client files
• Statistics: Maintain and report applicable statistics regarding programs and client services
• Patient Safety 1: Follow patient safety-related policies, procedures and protocols
• Patient Safety 2: Demonstrate proactive approach to patient safety by seeking opportunities to improve through questioning current policies and processes
• Patient Safety 3: Identify and report/correct environmental conditions or situations that may put a patient at undue risk
• Patient Safety 4: Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
• Patient Safety 5: Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they do not understand or may not seem right

Qualifications

• Education: Bachelor’s degree in a related discipline
• Experience: 1 year directly related experience
• Credentials: CPR for Healthcare/BLS Provider or Professional Rescuer within 30 days; valid New Mexico Driver’s License; UNM Vehicle Operator’s Permit within 60 days
• Physical Conditions: Light work: exerting up to 20 pounds of force occasionally, up to 10 pounds frequently, and a negligible amount constantly; may require frequent walking or standing or sitting with pushing/pulling of controls
• Working Conditions: Minor hazard—physical risks, dirt, dust, fumes, noise; tuberculosis testing upon hire and as required; subject to random alcohol and substance testing

• 17% for Weekday Nights
• 26% for Weekend Nights
• 15% for Weekend Day
